ABSTRACT

Objective@#To investigate the mortality and disease burden of diabetes in Hangzhou City from 2013 to 2021, so as to provide insights into improving diabetes prevention and control strategies.@*Methods@#Data on morbidity and mortality of diabetes in Hangzhou City from 2013 to 2021 were collected through the Chronic Disease and Death Cause Monitoring System of Hangzhou Center for Disease Control and Prevention, and the crude and age-standardized mortality (standardized by the population of the Seventh National Population Census in China in 2020) were calculated. Data on the prevalence of diabetes were collected from the China Chronic Disease and Risk Factors Surveillance (CCDRFS), and the burden of disease was evaluated by calculating years of life lost (YLL), years lived with disability (YLD) and disability-adjusted years (DALY) with reference to the methodology of the Global Burden of Disease Study (GBD). The trends in mortality and disease burden of diabetes were evaluated with the average annual percent change (AAPC). @*Results@#There were 8 686 deaths of diabetes in Hangzhou City from 2013 to 2021, with the average annual crude mortality of 13.17/105 and age-standardized mortality of 10.89/105. The crude mortality of diabetes showed a tendency towards a rise (AAPC=2.62%, P<0.05), while the trend in age-standardized mortality was not significant (P>0.05). The crude and age-standardized mortality of diabetes were higher in women than in men (14.11/105 vs. 12.21/105, 12.16/105 vs. 9.71/105; both P<0.05); however, the crude mortality in men increased more rapidly (men: AAPC=4.46%, P<0.05; women: AAPC=1.09%, P>0.05). The YLL, YLD and DALY of diabetes were 155.42, 1 246.73 and 1 402.15 thousand person-years, and the rates of YLL, YLD and DALY were 2.36, 18.90 and 21.25 person-years per thousand, respectively. The rates of YLL, YLD and DALY appeared a tendency towards a rise (AAPC=1.57%, 6.03% and 7.11%, all P<0.05). Higher rates of DALY and more remarkable increase were seen in men than in women (P<0.05; AAPC=8.37%, 5.54%, both P<0.05). @*Conclusions@# The mortality of diabetes in Hangzhou City from 2013 to 2021 is high, and the burden of disease, especially disability, is growing rapidly, with the burden of disease being more serious in men.

ABSTRACT

OBJECTIVES@#To study the clinical efficacy of ultrasound-guided endoscopic retrograde appendicitis therapy in children with appendix-related chronic abdominal pain.@*METHODS@#A retrospective analysis was performed on the medical data of 30 children with the chief complaint of chronic abdominal pain who were admitted from August 2019 to May 2021. All the children were found to have inflammation of the appendix or intracavitary stool and fecalith by ultrasound and underwent ultrasound-guided endoscopic retrograde appendicitis therapy. The medical data for analysis included clinical manifestations, endoscopic findings, white blood cell count, neutrophil percentage, length of hospital stay, and cure rate.@*RESULTS@#Among the 30 children with chronic abdominal pain, there were 13 boys (43%) and 17 girls (57%), with a mean age of (9±3) years (range 3-15 years) at diagnosis. The median duration of the disease was 12 months, and the median length of hospital stay was 3 days. The children had a median white blood cell count of 6.7×109/L and a neutrophil percentage of 50%±13%. Fecalith and a large amount of feces were flushed out of the appendix cavity for 21 children (70%) during surgery. The follow-up rate was 97% (29/30), and the median follow-up time was 11 months (range 5-26 months). Of the 29 children, abdominal pain completely disappeared in 27 children (93%).@*CONCLUSIONS@#Ultrasound-guided endoscopic retrograde appendicitis therapy is effective in children with chronic abdominal pain caused by feces or fecalith in the appendix cavity.

ABSTRACT

<p><b>OBJECTIVE</b>To establish the co-culture model of cancer cells and nerve, and to study the influence of esophageal cancer on nerve fibers.</p><p><b>METHODS</b>Mouse dorsal root ganglion (DRG) was cultured in sterile conditions by primary isolation. Co-culture model was established using matrigel matrix-embedded DRG and EC109 (esophageal cancer cell line) transfected with green fluorescent protein. Morphological changes of DRG, number and area of neurites were quantified with microscopy and image analysis. Furthermore, the mRNA expression of nerve growth factor(NGF) and brain derived neurotrophic factor(BDNF) was detected by real-time PCR.</p><p><b>RESULTS</b>In mixture cultivation model of EC109 and DRG cells, directional outgrowth of neurite projecting to EC109 was observed, and the length of neurite was markedly longer in proximal field compared to distal field. The number and area of neurite were 87 and 346 μm(2) in proximal field respectively, and 23 and 141 μm(2) in distal field on the 7th day. The expressions of NGF and BDNF were up-regulated in esophageal cancer cells.</p><p><b>CONCLUSIONS</b>The esophageal cancer may play an important role in nerve fiber growth and guidance, which may be associated with the up-regulation of NGF and BDNF expressions.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the general pattern of cholinergic nerve distribution and M(2) receptors in adult rat heart.</p><p><b>METHODS</b>Karnovsky-Roots histochemical staining combining point counting method and immunochemical SABC method with image analysis were used to identify the cholinergic nerves and M(2) receptors, respectively, in adult rat heart.</p><p><b>RESULTS</b>Positive staining of cholinergic nerves and M(2) receptors was found in all regions of the rat heart, and the point count of cholinergic nerves in the atria was 4.6 times as much as that in ventricles, and the area of immunoreactive substance for M(2) receptors two-fold higher in the atria than in the ventricles. The point counts of the cholinergic nerves in the medial-layer myocardium were fewer than that in subepicardial and endocardial tissues of the left ventricular free wall. However, M(2) receptors were comparable among the 3 layers of the left free ventricular wall.</p><p><b>CONCLUSION</b>Cholinergic nerves and M(2) receptors are located in both rat atria and ventricles, but their density is much higher in the atria than in the ventricles. Transmural heterogeneity characterizes cholinergic nerve innervation in the left ventricular free wall without significant differences in M(2) receptor density.</p>

ABSTRACT

OBJECTIVE: To systematically understand natural philosophical medical mode that is the important component part of theoretical medicine studies, to further explain its theoretical values and application significance, so as to promote the constant improvement of medical mode.DATA SOURCES: Based on Social Medicine---Evolution of Medical Mode---Empirical Medical Mode, synthesized the features of TCM, Chinese thought history, Chinese cultural history and the research method features, puts forward to the systematic understanding of natural philosophical medical mode.STUDY SELECTION: Data was selected from social medical data, TCM data, Chinese cultural data and thought history data. The selected data can embody outlook of nature, abstract thinking features and mode features. The data was analyzed and grouped to reveal the application of the thoughts and methods of natural philosophy in the understanding of health and diseases.DATA EXTRACTION: Among 23 papers, the data was mainly selected from 6 papers including Shanggu Tianzhen Lun, Shengqi Tongtian Lun and Zangqi Fa Shi Lun. The thoughts, theory and understanding from these materials are used to expound the natural and social phenomena of the human body, and get the conclusion that the qualitative analysis is sufficient, while the quantitative concepts are scanty; subjective experiences and clinical disproof theory are plentiful while concrete proof analysis and morphological injury principles are scarce.DATA SYNTHESIS: Based on the influences of holism and methodology of natural philosophy, the natural philosophical medical mode can only grasp the life phenomena organically. It attaches importance to the generalization of the relationships and phenomena above the level of the human body, but pays less attention to the concrete structure analysis below the level of human body. Therefore it emphasizes the interactive roles of the four-dimension structure of the heaven, earth, people and ethics in human health and diseases, invented many difficult material concepts to specify the physiological and pathological concepts. The life principles are invariable, the content of qi mechanism remains constant. All diseases can be understood, syndrome-differentiated and presc ribed. However the medical concepts are mostly cited from ancient philosophical concepts and the general level of medical theory remains in this level of thoughts.CONCLUSION: Natural philosophical medical mode centers upon social science, social phenomena and social psychology as its main tool. It attaches more importance to the essential attribute of man, namely the social attribute. By contrast the biological medical mode attaches importance to the natural attribute of man. namely the biological attribute. The two factors constitute the two sides of one thing. Compared with the present medical mode of biology---psychology---society, the natural philosophical medical mode not only has its systematic nature and holism, but also clinical therapeutic theory and more operational nature. It may enlighten people's wisdom and is an art of understanding health and diseases. Thus it possesses very important research significance and clinical application values.
